LM: Lateral Hip/Thigh Pain with Prolonged Walking: Initial Exam

  • Initial Visit: 6-9-17
    • Patient reports:
      • Walks 3-7miles per day especially while in Florida and walking on the beach
        • Possible contributing factor
      • Pain lateral L hip down the lateral leg and lateral calf
      • Wakes her at night with difficlty laying on it
      • Pain with walking, so she stopped walking
      • While in Florida got away from strength training and only did walking, possibly contributing
      • Has now returned to exercise
        • Leg press sometimes reproduces lateral knee pain
      • pain can radiate to medial joint line
      • Stairs, squatting, lunges
    • PMH:
      •  nothing significant
    • Physical Exam
      • Standing
        • elevation of L ilium
        • restriction in L shoulder flex
        • sidebending decreased L compared to the R
        • single leg balance L more unstable and lateral hip discomfort
        • collapse medial arch on the L
      • Supine
        • No significant mobility restrictions except
          • ankle dorsiflexion and gastroc mobility on Left compared to R
          • Mild restriction in L quad, hamstring and hip IR
      • Strength
        • Pain with squat with valgus collapse of bilateral knees
        • Significant inhibition of L glute max and bilateral glute med weakness
        • Bridge with mild R lateral knee pain
